Forms and Applications

Here you'll find forms and applications for short-term exchange, long-term exchange, and New Generation Exchange.

For all of the applications, please type them neatly and include a picture. Your host families will be seeing the applications, and this is their chance to learn more about you, so make sure you represent yourself accurately and fill out the forms completely.

Applications:

Please send one of the printed copies to Shannon Duckworth by email to sduckworth@wecpas4u.com or mail to P.O. Box 3718, Sonora, CA 95370 (209) 536-1218 . Please answer the essay questions using a word processing program and e-mail this as a separate file or you can print it out and mail it. The photos can be e-mailed as separate files or printed out and mailed. You also need to fill out the Choice of Programs Form.
